On arrival crews found fire showing in the rear of a two-story house. Crews made an offensive interior attack through the front door and conducted a primary search of the first and second floors. The occupants of the second floor stated they were awakened by a smoke alarm and saw the back room on fire and evacuated the building, the first floor was vacant and boarded up. Fire started coming through the roof and spreading through the attic space. The Incident Commander ordered crews out of the building and asked for a second alarm. On arrival, the Deputy assumed command and directed a defensive attack on the building. Possible cause of the fire is some incendiary device thrown at the rear of the building. Last company at scene at 0959. |

Companies were dispatched to a report of a structure fire and on arrival Engine 16 reported the front porch fully involved and laid 2 inch tank line. Fire was rolling up and out of the top of an unenclosed front porch, starting to spread to the second floor and it appeared that windows to the house had been broken from heat. MPD squad reported the back entry and porch area of the home were also fully involved in fire when they arrived via the alley. Engine 14 arrived and laid a supply line and a back-up line. Engine 4 assumed RIT. Ladder 4 raised its bucket. Engine 16 developed mechanical problems, which delayed water on the fire. Engine 4 backed down from 14th Avenue and laid a supply line to Ladder 4. Rescue 9 laid a 2 1/2-inch "blitz fire" line off Engine 16 to cover the exposure at 1418 Oliver. The fire spread rapidly through the structure forcing an exterior attack. Engine 16 and Ladder 4 reported water supply problems. A second alarm was requested and the Safety Officer set up a collapse zone. Engine 4 shut down and extended their supply line to Plymouth Avenue with assistance from Engine 1. Engine 2 laid their own supply line from 15th and Newton and down the alley to the rear of the structure and then laid their 2 1/2-inch "blitz fire" line. Engine 1 and Ladder 10 were used as relief on hand lines in the front of the building. The service door to the detached garage was found to have been forced open and inside there were two plastic gasoline containers, one set up with a paper wick stuck in the filler tube. The exposure at 1418 Oliver suffered water run off in the basement. Investigation determined the cause of the fire to be arson. The building was condemned and Engine 12 was called to the scene for standby while the building was torn down. Last company left the fire ground at 0836. |

Companies responded to a reported fire in a dry cleaning & laundry establishment. On arrival firefighters found fire in the rear, primarily in a storage shed that was attached to the building. After forcing entry, fire was found in the ceiling of the shed that was extending to the interior of the building by spreading upward and into the eaves of the main building. An interior attack was attempted but due to a possible false ceiling area and rapidly changing conditions the attack was changed to defensive and master streams were set up and exposures covered. When the major volume of fire was knocked down, hand lines were used to get at the remaining hot spots. A melted gas container was found outside the shed in the area of origin. Last company left the fire ground at 1157. |

On August 1, 2007, the Minneapolis Fire Department responded to numerous 9-1-1 calls reporting the collapse of a bridge over the Mississippi River. Upon arrival crews reported a major collapse of the Interstate 35W Bridge and initiated the request for additional resources. Approximately 28 pieces of MFD apparatus responded to the call, which included engine companies, truck companies, command vehicles, technical rescue vehicles, hazmat teams, and water rescue equipment. A general callback of all department personnel was requested through dispatch in addition to a mutual aid request for support from mutual aid partners. In response to the general callback, personnel returned to their assigned stations to staff spare apparatus and assist mutual aid partners who were requested to back fill stations for units that responded to the incident. On arrival crews engaged in immediate rescue efforts. Rescue operations were focused in two divisions that were established on the North and South sides of the bridge collapse including a Water Group that assumed the water rescue responsibility on and around the portion of the bridge in the river. Initial Incident Command was established on the 10th Avenue Bridge to direct the immediate rescue actives. The 10th Avenue Bridge gave Command a very good vantage point to observe the rescue efforts in both divisions and the water group. Staging areas were established by Command for arriving support units and personnel from local, county, state, and federal agencies.
The initial rescue efforts were very challenging under very tenuous conditions with the risk assessment focused on the unstable conditions and the potential for secondary movement collapse of additional bridge sections. Structural engineers were requested early in the incident to make an assessment of the collapse site in regards to the potential of a secondary collapse and to assist with the risk management of responders. Personnel assigned to each division faced many challenging rescues and extrications. They faced the task of assisting multiple victims who had self-rescued prior to arrival of responders. The South Division was confronted with several vehicle fires requiring multiple lines to be laid along with an elevated master stream to gain control and extinguishment of these vehicle fires. The unstable conditions caused by the collapse and the position of some of these vehicles made all fire suppression and rescue efforts very difficult. The North Division faced many of these same conditions including reports of rail cars containing hazardous materials, power lines down under the collapse, and the report of a facility containing radioactive isotopes being damaged with the potential for possible contamination. Command isolated the identified hazards and made requests for assistance through Unified Command for mitigation assistance. The Water Group rescued victims from the portion of the bridge decking located in the river and transported victims to triage and treatment areas established in both divisions. Emergency Medical Services led by Hennepin County Medical Center set up triage, treatment. and transport areas in both the North and South divisions. There were four such areas established due to access issues created by the collapse of the bridge. The EMS system did a remarkable job in triage, treating, and transporting approximately 55 patients in 1 hour and 40 minutes from the time the first call was received.
At the completion of the initial rescue phase the Command was relocated to the Unified Command Post location that was comprised of cooperating agencies from local, county, state, and federal agencies. As the incident moved into the recovery phase, the fire department passed command to law enforcement and assumed a support role throughout the remainder of the recovery effort. Command of Tactical operations was confronted with a number of focused areas during the rescue phase of the incident including, Life Safety, Fire Hazmat, Technical Rescue, Water Rescue, Safety of Response Personnel, Accountability, and Security. The last MFD unit cleared the scene on August 2nd at 1920 hours.
Editors Note: After the initial Box alarm, additional requests for companies were made until the arrival of the Deputy Chief at 1825hrs. At that time the Deputy asked for ALL city FD companies to respond, essentially a General Alarm. Chief of Department Clack modified the request for all remaining companies to respond at once, to one where they were all to respond, but they were to be dispatched in a staged manor. While no official ‘extra alarms’ were struck, but since all companies responded, and a ‘general alarm’ equivalent was issued, the Extra Alarm Assoc. has assigned this incident a 4-4 alarm (a 4.75 alarm) assignment, the highest currently available. |

First alarm assignment was dispatched to a report of smoke coming from the roof of a large church on the corner of Malcolm and Orlin. On arrival Engine 19 found light smoke from the roof and began to investigate and forced entry to the front door but found no smoke or heat inside the church. The church, with a two-story atrium, was under construction including no floorboards in the two-story atrium. All crews began working to identify the location of the fire and the best means of access for attack. Rescue 9 attempted to cut a hole in the exterior wall off the aerial while Ladder 3 used ground ladders in the rear to begin roof ventilation. Ladder 3(a) used a ground ladder in the interior to reach the ceiling of the atrium and expose the fire. Engine 19 stretched a regular line to the interior to attack the fire from this vantage point. Rescue 9 was unable to access the concealed space from the exterior due to concrete siding. A second alarm was struck for relief crews due to the heat. Last company left the fire ground at 0806. |

A first alarm assignment was dispatched to the report of a building fire. First arriving Battalion 4 found a two-story single-family house with heavy fire on the first and second floor already venting out the south side. There were exposures on both the north and south sides with the south side exposure being a vacant single-family house beginning to smoke from the venting fire. Engine 16 laid a tank line through the front door and extinguished the fire on the first floor. They then proceeded to the second floor and encountered a large hole in the floor from the fire burning through. Ladder 10 arrived and set up for defensive protection of the south exposure. There was a partial roof collapse from the free burning fire. Ladder 10 and Rescue 9(a) completed a primary search of all floors. Rescue 9(b) laddered all sides on the building for egress and assumed RIT. Engine 14 was laying a supply line and a back up line but encountered a broken gate and had to switch at the hydrant with Engine 4 for a supply line. Crews were withdrawn from the building due to the delay in water supply and worked defensively to knock down the fire and protect the south exposure. A second alarm was stuck with staging on Russell Avenue and 16th. Once the fire had been somewhat knocked down from the exterior, crews then returned to an offensive attack. Second alarm companies were used in relief and overhaul. Last company left the fire ground at 1027. |

An initial assignment was dispatched to 1717 Washington Street Northeast, a 17-story hi-rise apartment building. On arrival Battalion 4 reported black smoke showing from a 9th floor window on the north side of the building, assumed command, and requested a full first-alarm assignment. After first alarm was assigned a second alarm was requested with staging at 19th and Washington Street. On arrival Engine 15 received a report from residents that the fire was in apartment 1002 on the 10th floor with a possible occupant inside. Engine 15 took the elevator to the 8th floor and walked up to the 10th floor with hi-rise bundle reporting smoke in the 10th floor hallway and requested Ladder 4 to bring up apartment keys from the outside key box. Engine 11 arrived and proceeded to the 8th floor with hi-rise bundles. Ladder 4 retrieved apartment keys and went to the fire floor for search and ventilation. Ladder 4 forced entry to the apartment and reported a couch fire and knocked down fire with a pump can. The tenant of apartment 1002 was not in the apartment at the time of the fire. Engine 15 hooked up an attack line on the 10th floor in stairwell "A" and Engine 11 hooked up a backup line on the 9th floor in stairwell "B". Rescue 9 was assigned to the 10th floor to assist with ventilation, evacuation, and searches. Engine 15 used attack line to fully extinguish the fire. When the fire was placed under control second alarm companies were released. Crews continued to overhaul and ventilate. Last company left the fire ground at 1918 hours. |

A first alarm assignment responded to a reported building fire at 2317 Fremont Avenue North. Engine 14 reported heavy smoke from the rear of the second and third story of a large residential home and laid a tank line up the rear stairwell for an offensive interior attack, reporting a strong smell of gasoline in the stairwell. Ladder 10 assisted in laying line and conducted primary searches with Rescue 1. Engine 4 laid a back-up and supply line. Engine 20 assumed RIT and threw egress ladders. Crews knocked down the fire on the second floor and the bulk of the fire on the third floor, but due to spongy floor, holes, and difficult access, they were unable to reach the front half of the third floor where the majority of fire was located. A second alarm was struck and crews were pulled out when fire broke through the front roof area. Engine 14 and Ladder 10 used master streams to achieve a quick knockdown of the third floor fire. Crews reentered the building, extinguished remainder of fire, and performed extensive overhaul. Last company left the fire ground at 21 26. |

Engine 4 was sent alone to check on a report of wires down at 25th and Aldrich Avenues North. While enroute, dispatch reported they were getting several calls related to these wires and then reported they were now getting a report of a house on fire and started a first alarm assignment. Engine 4 arrived found fire showing from the first floor rear of a two-story house with heavy smoke coming from the second floor. Engine 4 laid a tank line for an interior attack. While attempting to gain access to the house from the side door, Engine 4 found an elderly man standing next to that door and carried the man out to the street. Battalion Chief 4 arrived at that time, saw Engine 4 carrying the man, and requested an ambulance and had the man placed in the back seat of his vehicle. Ladder 4 arrived, assisted Engine 4 with forcing entry, and conducted a primary search of the first floor. Engine 2 laid a supply line and a back-up line. Battalion Chief 1 arrived and reported that the power pole to the house was down in the back and was arcing and that the first floor windows had wire mesh on them. Rescue 9 was assigned exterior ventilation. It was discovered that the windows had 1/4-inch wire mesh, then plywood, and then Plexi-glass Lexan on all first floor windows, which made ventilation extremely difficult. The front door had three or four dead bolts and the side door forced by Engine 4 had three dead bolts. Engine 14 assumed RIT and attended to the elderly man removed by Engine 4 who was later transported to hospital by North Ambulance. Engine 4 reported they had the bulk of the fire knocked down on the first floor and told Engine 2 to lay their line to the second floor. Crews reported a lot of clutter in the house and were having a hard time moving around to perform overhaul. Ladder 4 went to the roof for a ventilation hole. A second alarm was requested after crews had gone through two SCBA bottles. Engine 16 was assigned RIT. Ladder 10 went to the second floor and on the way up they reported heavy fire in the first floor ceiling. At that time the order was given to evacuate the building. Engine 14 laid a supply line to Ladder 4's water tower. Engine 11 laid their own supply line and used a master stream and a blitz-fire line. Engine 4 and Engine 14 both laid blitz-fire lines off Engine 4. Once the bulk of the fire was knocked down, all lines except for Ladder 4 were shut down and picked up. Engines 17, 22, and Ladder 11 were sent in as relief crews. A tree falling on the electrical service mast to the house caused fire. Last company left the fire ground at 0804. |
