An initial alarm assignment was sent to check the report of an explosion and fire in a manufacturing facility and called for a full first alarm assignment on arrival. Engine 11 reported an explosion in a machine doing a roll forming welding coating process had injured a worker and started a fire. Engine 11 laid a pre-connect line to the interior with a supply and back up line by Engine 19. Ladder 3 and Rescue 9 checked on extension of the fire and found fire had extended to the wall and roof area. Engine 1 laid a line to the interior while Ladder 3 laddered the roof. A second alarm was called to cover any further extension. Engine 7 assumed RIT with other companies held in staging. A 65-year old worker suffered severe burns and was transported to hospital by ambulance along with another worker who reported ringing in the ears. Last company left the fire ground at 1144.

Engine 8 arrived reporting fire and heavy smoke showing from a 2-1/2 story residential building with three victims trapped on the second floor roof and an exposure problem in sector D. Engine 8 began an interior attack through the front door while Ladder 3 and Rescue 1 laddered the building and rescued the three victims from the roof. A second alarm was called while Engine 5 laid a supply line and back up line. Engine 17 assumed RIT. Ladder 5 and Engine 21 checked for extension. Fire was contained to the first and second floor front. Two adults and three children were taken to hospital for observation of smoke inhalation. One firefighter was taken to hospital for a hand injury, and another was treated for an ankle injury. Last company left the fire ground at 2102.

Engine 1 reported heavy smoke on the second floor and was going to lay into the standpipe, but found no standpipe. Engine 7 brought in a tank line through the fire door on the 5th Avenue side and Engine 1 brought in a back up line. Ladder 11 forced entry to apartment and found bathroom fully involved including a mattress in the bathtub. Ladders 11 and 3 and Rescue 1 performed primary and secondary search, Engine 6 assumed RIT, Engines 2 & 11 were released. Damage was confined to the bathroom with some extension to the living area. One firefighter was treated for a head injury. Last company left the fire ground at 1351.

Initial assignment was sent to check a report of smoke from an apartment. Engine 4 arrived and was met by building security personnel who reported smoke coming from apartment 707. Engine 4 and Ladder 4 secured the elevators and went to check. Engine 4 asked for a full assignment due to apartment door being hot to the touch. Engine 4 hooked to a stairway standpipe while Ladder 4 opened the door to the apartment. Encountering heavy smoke, Engine 4 requested a second alarm. Engine 16 proceeded up with their high-fire equipment. Engine 4 hooked to the exterior connection and received a supply line from Engine 16. Engine 4 reported a mattress fire with extension to a chair. Ladder 4 did a primary search of the apartment and found it to be unoccupied. Engine 14 assumed RIT and Rescue 9 was sent up to ventilate and check for extension and water damage. There was no fire extension beyond the room of origin and no water leakage into the apartment below. After determining there was no need to evacuate the seventh floor, second alarm companies were sent home. Last company left the fire ground at 0445.

Companies arriving on a reported structure fire reported heavy smoke showing from a large one-story auto body repair shop. First arriving battalion chief ordered all companies to go into a defensive attack mode in order to protect a large two-story residential over commercial complex in sector B and a one-story garage in sector C, and then requested a second alarm to assist in protecting exposures. Second floor residents of exposed structure in sector B were evacuated and an MTC bus was requested to temporarily hold approximately 50 displaced residents. Ladder 4, an aerial ladder platform with two master streams, was set up in sector B and supplied by Engine 1. Engine 15, an aerial ladder tower, was placed in sector A and given primary task of extinguishing original structure fire. Engine 2 supplied Engine 15 and also a ground monitor in sector C. Ladder 3 and Engine 19 set up a water tower in sector C. Fire under control at 0357 with last company leaving the fire ground at 0727.

On arrival Engine 1 reported smoke showing from a 2-story commercial building and up-graded the assignment to a first alarm. Engine 1 initiated an offensive interior attack and laid a tank line to the second floor. Ladder 11 forced entry and started a primary search of the second floor. Engine 6 laid a supply line and a back up line. Rescue 1 went to the roof and cut a ventilation hole. Engine 7 assumed RIT. A second alarm was called and Ladder 4 assisted in overhaul and Engine 5 relieved crews on the second floor. Secondary searches were completed and all was clear. The building was ventilated and secured. Last company left the fire ground at 1037.

An initial assignment was sent to 315 East 15th Street and on arrival was given a corrected address of 315 West 15th Street. On arrival crews found smoke on the third and fourth floors of a four-story apartment building and Battalion 1 requested a full first-alarm response. Fire was found between the third and fourth floors with some extension and a second alarm was requested. Primary search of the building was conducted and all residents were out of the building. Extension of the fire in the attic was found and a third alarm was called. A firewall kept the fire in the attic space to the B-side of the building. Extensive overhaul was needed to extinguish fire in the attic and void space between the third and fourth floors. Last company left the fire ground at 0314.

Fire started in the kitchen of one of the two second floor apartments. Occupant was heating oil on an electric stove when the oil exploded, blowing flames toward the ceiling, and eventually spreading to the porch and into the adjacent apartment. First arriving companies reported smoke and fire showing from the rear of a two-story occupied structure. Engine 8 and Engine 7 initiated an offensive interior attack to the second floor. Ladder 3 and Rescue 1 initiated search, rescue, and ventilation operations while Engine 17 assumed RIT. Crews encountered heavy heat and fire on the second floor. Chief 2 ordered the evacuation of the building and requested a second alarm and conducted a PAR check. Crews went to an exterior attack and knocked down the majority of the fire and returned to an interior attack to extinguish the remaining fire. One firefighter was transported to the hospital with a back injury. Damage was estimated at $200,000 and companies remained on the scene overhauling until 1349 hours.

An initial assignment was sent on reports of a gas leak next to a commercial building. On arrival, crews found heavy smoke coming from a large one-story commercial building and asked for a full assignment. Crews entered the building to find the seat of the fire. On his arrival, the Deputy ordered a second alarm. It was determined that the fire involved the electrical/mechanical system and crews were removed until the power could be cut to the building. Crews ventilated on the roof and used a line to extinguish the fire after the power was cut. Damage was estimated at $25,000. Crews remained on the scene until 1125 hours.

Crews summoned to a reported structure fire found fire showing from the first floor of a two-story frame duplex. Engine 17 laid a tank line through the side door for an interior attack. Ladder 5 forced entry and did a primary search of the building and did ventilation. The fire was spreading fast from the front to rear and from first to second floor and a second alarm was requested. Crews were forced to take a defensive mode and protected an exposure on the "B" side. A deck gun, master stream and a water tower were put into operation to protect the exposures. The fire was quickly put under control. One firefighter received a second-degree burn on his hand. Damage was estimated at $200,000 and companies remained on the scene until 0858 hours.

This fire involved fire in a stove on the third floor or a high-rise with many elderly residents. Engine 21 found smoke showing on arrival and laid a line into the standpipe. A second alarm was sounded. The fire was quickly extinguished with sink water and Ladder 3 ventilated the smoke from the unit. Second alarm companies were staged and returned. Damage was estimated at $500 and companies cleared the scene by 1652 hours.

Firefighters were summoned at 0229 hours to Lowry and Central for fire reported in a blue building. Firefighters could not find the reporting party or any smoke or fire and returned to quarters. About 15 minutes later, a TV news van parked nearby, reported seeing flames from the rear of the building and firefighters were sent back to the scene. (Publisher's note: There are unofficial reports that this fire may have been set by an arsonist- two times.) Crews returned to the scene and found a large amount of fire on the rear-wooden deck of a two-story building and heavy fire from the first floor of an attached structure. Engine 15 made an interior attack in 2419 Central Avenue NE and Ladder 4 searched 2419 and 2421 Central in the apartments over commercial businesses. Engine 2 laid a back-up line and Engine 11 assumed RIT. A second alarm was requested. Second alarm Engine 1 laid a line for an interior attack on 2421 Central and was supplied by Engine 19. Ladder 3 was set up in the rear alley. On the arrival of the Deputy Chief, the two-story deck was fully involved and fire had broken through windows in the 2421 building. Interior lines were ineffective as the fire was making rapid progress. Upon completion of the primary search, the building was evacuated and crews went into a defensive attack. A third alarm was requested, with special calls for additional apparatus. Master streams were set up using Ladder 4's tower, Ladder 3, Engine 15 (quint), Engine 11 (quint) and Ladder 11's tower. A collapse zone was set up around the exterior of the building. Partial collapse occurred in both sector "B" of 2421 and sector "C" of the 2417 building. The fire eventually consumed three of four buildings, with extensive smoke and water damage in the fourth building. Three vehicles were also destroyed in this fire. One firefighter was transported to the hospital with and ankle injury and one firefighter was treated for back strain. Damage was estimated at $791,000 and crews remained on the scene until 1824 hours.

Engine 14 arrived to find the fire ruins of a vacant boarded building fully involved with fire coming out of the roof. Engine 14 laid a tank line to cover the exposure building and then set up a master stream on the fire building. Engine 4 set up a master stream and a hand line. Chief 4 called for a second alarm for stand-by companies. Companies continued to fight this fire in a defensive mode until the roof and second floor were darkened down. Rescue 9 and Ladder 4 checked the building for structural integrity before crews entered for an interior attack. Although the building had been boarded up from a previous fire, it was found that a window next to the south side door had been pried open. Last company left the fire ground at 0756.

A first alarm assignment was dispatched to a possible fire at a coal burning electrical generating plant. First arriving company reported smoke or steam showing from an upper floor and was going to investigate. Chief 4 arrived, requested a second alarm, and set up a staging area. Information was received that the incident was on the ninth floor. Ladder 10 and Rescue 9 were assigned to assist in the evacuation of the eighth and ninth floors and an engine company was assigned as RIT on the seventh floor. It was discovered that the problem was a broken high-pressure steam pipe and maintenance was shutting down the boiler that fed the steam line. Last company left the incident at 1529.

Police on scene reported a building fully involved across the street from 3010 Penn Avenue North. Engine 14 observed large fire while enroute. On arrival Engine 14 reported building fully involved with exposures on the north and south sides, 3023 and 3031 Penn Avenue North. Engine 14 laid a forward lay to the fire and set up deck nozzle plus hand lines with Blitz Fire Nozzles. Ladder 10 arrived and started evacuation of 3023 Penn, a two-story multi-family dwelling. Fire building was totally collapsed into itself. Engine 16 was placed in front of 3031 Penn and set up their Squrt for use on 3027 Penn and to protect exposure on the north side. Engine 20 laid a supply line to Engine 16 and a hand line off of Engine 14 to protect the interior at 3023 Penn. Rescue 9 laid a hand line off of Engine 14 to cover the exposure at 3031 Penn. Engine 14 could not support any more lines and a second alarm was struck for staging at Penn and Lowry Avenues North. Engine 4 arrived and laid a forward line from Lowry Avenue to behind Engine 16 to cover 3031 Penn as the vinyl siding was melting and sagging. Engine 20 was able to conduct a search in 3023 Penn and look for extension of fire. The building was clear with no interior extension, however there was damage to the siding and windows. Engine 4 conducted a search and check for extension in 3031 Penn. Building was clear with no interior extension, however there was damage to the siding and windows. Engine 2's crew set up a hand line for an interior attack into 3031 Penn if needed. Between Engine 14's deck gun and Engine 16' Squrt the bulk of the fire in the rubble at 3027 Penn was knocked down and fire was placed under control. Last company left the fire ground at 0455 on November 18.

An initial assignment was sent to check on a report of smoke coming from a house at 2528 West Lake of the Isles Parkway. On arrival, Engine 22 found heavy smoke but no fire showing and upgraded the run to a full first alarm assignment. Engine 22 forced entry into the front after attempting to do a "360" but was blocked by a wood gate on one side and a retaining wall in the rear. Engine 22 laid a tank line into the front door while Engine 6 laid a back up line and a supply line. Ladder 11 began searches on the first and second floors. On entry, crews had trouble finding the fire due to the smoke condition and the unusual floor plan and the size of the house (83' x 80' three-story unoccupied dwelling). Rescue 1 assisted Engines 22 and 6 laying lines into the front for an interior attack. Engine 6 with thermal imaging equipment still had trouble finding the fire. Fire was found at the rear of the building and lines were re-directed to the rear with Engine 8 assuming RIT. On arrival of the Deputy, heavy smoke was showing from vents on the front of the house in the cockloft and between the first and second floors. Crews had found and were attacking the fire at the rear of the building. Crews were reporting the bulk of the fire had been knocked down and were leaving the building for a bottle change. On re-entry, they found fire in the walls on the first and second floors. Rescue 1 reported heavy fire in the cockloft and high heat readings in the walls. Several crews reported heavy labor required to open walls and ceilings. Conditions changed rapidly with reports of fire on all three floors and in the cockloft. The primary search of all three floors had been completed and command made the call to go on a defensive attack at that time. Throughout the night and into the following day various companies were rotated in as relief. Last company left the fire ground at 1521 on December 1.

First alarm assignment was dispatched to a report of a structure fire at 2701 Harriet Avenue South. Engine 8 arrived and found fire showing from a basement window of a two-story, 40' x 100', occupied multiple family building. Engine 8 laid a tank line through the front door for an interior attack. Engine 6 laid a supply line to Engine 8 and a back up line. Rescue 1 started a primary search of the fire floor and assisted Engine 8. Ladder 11 took one individual out of a top floor window in the rear. Engine 5 assumed RIT. An unknown male removed two victims through a rear door. A second alarm was struck with staging at 27th and Garfield. Engine 17 was used for ventilation and overhaul, Ladder 5 was used for overhaul, and Engine 22 remained in staging. Primary and secondary searches were completed. Three female victims were transported to hospital and treated for smoke inhalation. One firefighter suffered minor injury from a fall and was treated at the scene. Last company left the fire ground at 0724.
